Beth Emhoff rejser hjem fra Hongkong til Minneapolis med en hidtil ukendt og ekstremt smittefarlig virus i kroppen. Efter to dage er både hun og familiens søn død. Også i London, Tokyo og Chicago falder folk som fluer. Pandemien breder sig over kloden som en steppebrand. Mens researchere kæmper for at finde en modgift, bidrager bloggeren Alan til panikken ved at sprede misinformation på internettet.

De får jaget den makabre fremmede væk, men for sent. En dødelig og kødædende virus har forplantet sig i flokken, og da de første symptomer begynder at vise sig, bredes panikken som ringe i vandet. Medlidenheden bliver lynhurtigt vendt til afsky, frygt og væmmelse, men selvom første offer aflåses i et skur, truer det psykologiske nedbrud, og resten af den panikslagne flok må nu gøre hvad de kan for at overleve både virussen, og hinanden...

En ensom rejsende fra år 2035 skal løse en gåde, der kan redde hans folk. men den kan også føre ham til afgrunden af sindssyge. Efter at det meste af verdens befolkning er blevet udslettet af en dræbervirus, må de overlevende bo i små samfund under jorden. Cole 'melder sig frivilligt' til at rejse tilbage til fortiden for at skaffe en ren virusprøve, så man kan fremstille en kur. Undervejs møder han den smukke psykiater og den næsten sindssyge. Men tiden er kort, mens Cole leder efter The Army of the 12 Monkeys, en yderliggående gruppe med forbindelse til den dødelige sygdom.

Så er den gal igen! Menneskeheden er ved at blive overtaget af en ondsindet rumvirus, der inficerer alles sjæle i søvne og forvandler dem til følelseskolde hylstre.
