Boksačke dane je ostavio iza sebe, potratio svoj talent i zaglibio u problemima s drogom. Njegov mlađi brat Micky u međuvremenu je postao boksač, ali ne s izglednim šansama za osvajanje naslova prvaka. Unatoč svom trudu, Micky gubi borbu za borbom. Nakon tučnjave u Moskvi liječnici otkrivaju da ima oštećenje mozga i Rocky se ne smije više boriti. Ubrzo proda kuću i s obitelji se vrati u stari siromašni kvart. Ondje trenira Tommyja Gunna (Tommy Morrison), mladog boksača koji ga napusti i udruži se sa Georgeom W. Dukeom (Richard Gant). Kako bi zaradio poštovanje obožavatelja, Tommy se želi tuči s bivšim prvakom. No to bi mogla biti Rockyjeva zadnja borba.

Kada se Rockyju učini da je situacija postala bezizlazna, stiže mu neočekivani poziv na revanš s Apollom, koji se ne može pomiriti s ishodom prvog meča te pričama kako je rezultat bio namješten. Adrian se toj ideji oštro protivi, kao i doktori koji upozoravaju Rockyja kako bi borba mogla imati pogubne posljedice po njegov vid.
