L'ispettore Fein dovrà indagare sull'omicidio di un prete russo, che si era immischiato nell'organizzazione criminale russa. Però, viene aiutato da Ben e Eddie per le indagini.

In un quartiere di New York si stanno verificando numerose sparizioni, soprattutto di barboni, apparentemente senza motivo. Il capitano Bosch (Christopher Curry), dopo che anche sua moglie è scomparsa, comincia a investigare sul caso. Alle indagini prendono parte anche il "Reverendo" (Daniel Stern), un ex detenuto che gestisce una mensa per poveri, Cooper (John Heard), che in passato aveva realizzato un servizio fotografico sui senzatetto che vivono nel sottosuolo e la sua ragazza (Kim Greist). Gli indizi portano tutti alle fogne, dove dei terribili esseri mutanti rapiscono e uccidono tutti coloro che gli capitano a tiro.

Tutto fa pensare a un complotto mafioso ai danni di Scarfia. Le indagini si fanno di giorno in giorno più complesse e la cronista e anche la polizia sembra brancolare nel buio.
