Hành trình kết nối những trái tim là chương trình truyền hình thực tế do HTV, HanoiTV và MCV phối hợp sản xuất cùng với nhà tài trợ Doublemint. Chương trình được sản xuất dựa trên bản quyền chương trình Ainori hay Love Ride của đài truyền hình Fuji, Nhật Bản. Chương trình gốc đã có hơn mười năm hoạt động, tập đầu tiên ra mắt ngày 11 tháng 10, 1999 và tập cuối cùng phát sóng ngày 23 tháng 03, 2009, tổng cộng có 441 tập phim nhưng vẫn thu hút được một lượng lớn khán giả và hiện đang nằm trong tốp những chương trình truyền hình có tần suất khán giả đón xem cao nhất. Hành Trình kết nối những Trái tim bắt đầu khởi quay ngày 17 tháng 10, 2008 và lên sóng chính thức ngày 02 tháng 12, 2008. Chương trình được phát sóng vào lúc 22:00 vào các ngày thứ ba trên HTV7 và 21:30 trên HanoiTV. Cùng với HTV7 và HanoiTV, 15 đài truyền hình địa phương sẽ tiếp sóng và phát lại. Hiện tại, đây là chương trình truyền hình thực tế được khán giả qua tâm nhiều, thu hút người xem đài Việt Nam.
